The Sutter Continuum Case Management department provides coordination of services to help patients maintain the best health possible. In 2003 the department was awarded the inaugural Franklin Award of Distinction by the Joint Commission of Accreditation of Healthcare Organizations (JCAHO) and the American Case Management Association for improving quality across the continuum of care. Sutter's Continuum Case Management Department also offers special resources for Sutter hospital patients and those who need extended care after they leave the hospital.
